Glitch Global Group is an upcoming assisted living facility in Jacksonville, FL, founded by Registered Nurse NuKarriuss Lewis. Opening Spring 2027, we are dedicated to providing elevated, home-like assisted living where comfort, elegance, and compassionate care unite. Our mission is to create a sanctuary for seniors that honors their dignity while providing the professional support they need. We are committed to regulatory compliance and are pursuing licensure as a Florida Assisted Living Facility under the Agency for Health Care Administration (AHCA).
Our intimate 6-bed facility features 24/7 staffing to ensure residents receive personalized attention and care. We specialize in reducing hospital readmissions through proactive health management and compassionate support. Every aspect of our service—from assistance with daily living activities to medication management—is designed with the resident’s well-being and comfort in mind. Our Services
Readmission-Reduction Focus
The facility has a focus on ongoing monitoring and early intervention to help minimize preventable hospital readmissions. This includes regular health checks and proactive management of health conditions.
Discharge-Ready Admissions
The facility has structured intake processes designed to support timely hospital discharges and reduce avoidable inpatient days. This includes comprehensive pre-admission assessments and coordinated transition planning to ensure a smooth transition for residents moving from a hospital setting to the facility.
Nutrition & Meals
The facility provides three daily meals, including snacks. Special diet accommodations are available for residents with specific dietary needs (i.e., diabetic, cardiac, renal, easy-to-chew). The facility also has a dietician-informed meal planning process and monitors hydration to ensure residents receive proper nutrition.
24/7 Care and Support
Round-the-clock staffing with professional caregivers available at all times to assist with daily activities, emergencies, and personalized support tailored to each resident’s needs.
Medication Management
Expert medication administration and management services provided by our nursing staff to ensure proper dosing, timing, and monitoring of all medications for optimal health outcomes.
Housing & Utilities
The facility provides full-time living arrangements with thoughtful amenities in a home-like environment that is least restrictive. This includes comfortable accommodations, utilities, and maintenance services to ensure a safe and comfortable living environment for residents.
Assistance with Daily Living Activities
Comprehensive support with activities of daily living including bathing, dressing, grooming, mobility assistance, and meal preparation in a comfortable, home-like environment.
